I think that referee should also be fined/disciplined. He was literally looking right at it less than ten feet away. Nodding his head at the Canucks’ plead for an obvious call, but none was forthcoming. Wow.

Steve's right, a fine – especially the paltry maximum they are allowed to be fined – is no punishment at all. When they say "you've been fined five thousand dollars", the players hear "we have a sale on glove-on punches to the face today. You're allowed to do it for the price of five thousand dollars."
